Study Abroad Application Forms
There are several forms that must be completed in order to study abroad through Millikin University. You can pick up the forms at the Center for International Education in University Commons, Room 122. The forms you'll need to complete include:
- Registration form, for semester abroad only
- Study Abroad Pre-Registration Form
- Study Abroad Application
- Study Abroad Information Sheet
- Study Abroad Statement of Responsibility, Release, and Authorization
- Study Abroad Recommendation Form
- International Fellowship Program
- International Fellowship Program Recommendation form
- Emergency contact information
- Statement of Responsibilities
- Indemnification Wavier (which must be notarized)
- International insurance form
- International Student Identity Card (ISIC) Application
- Recommendations, for full-semester abroad programs only
- Waver, Release, and Indemnification Agreement
All students who are studying abroad through Millikin, whether its for a full semester or a short immersion course led by an MU faculty member, you must fill out all of the required paperwork.
